- The episode title is a reference to the Last Man Standing challenge and the TV series Last Man Standing.
- In the couch gag, Homer says "this guy must be a pinball wizard," referencing the song "Pinball Wizard" by The Who.
- Lenny asks if a woman named Diane worked at Moe's Tavern, but got replaced by a woman named Rebecca. This is a reference to Diane Chambers and Rebecca Howe from Cheers.
The Danish drug Othinquic takes a leading role in the episode. It is a parody of Ozempic, an anti-diabetes and anti-obesity medication sold by the Danish multinational Novo Nordisk
