He almost always succeeds in his attacks against Scratchy, and typically ends up killing him. There is only one known episode where Scratchy wins and Itchy is killed.

Itchy's first in-universe appearance was in cartoon "[[Manhattan Madness]]" under the name "Itchy the Lucky Mouse", where Itchy strolls around Manhattan, kills an Irish man, and then Teddy Roosevelt.
